TISと東和薬品、デジタルヘルスケアパスポートを発表

TIS Inc., a member of the TIS INTEC Group, and Towa Pharmaceutical Co., Ltd. TIS has launched the ‘Healthcare Passport’ at Chiba Neurosurgical Hospital in Chiba City. The initiative gives patients a digital record of their medical history, called their patient journey. Hospital pharmacists create this record to support better team care, even after patients leave.

The ‘Healthcare Passport’ is a personal health record (PHR) platform by TIS. It lets patients share their health and medical info safely with doctors and family. The system lets users safely share data through a cloud platform. It ensures two-way communication and easy access.

断片化された患者情報という課題への対応

Chiba Neurosurgical Hospital believes it’s important for patients to know their complete treatment history. Since 2019, the hospital’s pharmacists have created paper ‘medication information sheets.’ These sheets outline each patient’s treatment journey. They are provided at discharge. These documents help patients tell other doctors about their medications and treatments. This improves their care continuity.

Paper records are tough to store. They can get lost easily. Plus, sharing them with distant family members is hard. To fix these issues, the hospital chose to digitize the system. In the summer of 2024, it adopted the ‘Healthcare Passport.’ This decision was made with Towa, its pharmaceutical partner. They chose the platform because it offers secure, cloud-based sharing features.

試験的な導入とエンゲージメント

A demonstration project kicked off at the end of 2024. It started when discharged patients installed the ‘Healthcare Passport’ app. Initially, pharmacists assisted with installation when patients left the hospital. Later, the hospital shifted to home-based setups by family members. This change eased staff workload and made sure patients still consented.

2025年5月までに、対象となった患者の約40%がアプリをインストールしました。これは529人中212人です。患者は、その利便性、携帯性、災害時、特に病院の記録に到達するのが困難な場合の有用性を評価しました。家族は採用を支持しました。オンラインで病歴を閲覧でき、ケアプランに参加できることが好評でした。
